Q1 · Zoology · Human Health and Disease
Chronic use of alcohol can damage the liver, causing:
Answer: (C)
The chronic use of drugs and alcohol damages the nervous system and the liver (cirrhosis). The use of drugs and alcohol during pregnancy is also known to adversely affect the foetus.

A community's drinking water becomes contaminated, and many people develop severe watery diarrhoea that leads to dehydration. Which organism is most likely responsible, and what is the most appropriate immediate treatment?
Answer: (A)
Principle: cholera is a bacterial disease caused by Vibrio cholerae, spread through contaminated water. Its toxin causes profuse watery diarrhoea and dehydration. The immediate treatment is oral rehydration, which replaces fluid and salts. Students should combine the organism identification with the treatment logic.

A patient receiving chemotherapy for a solid tumour develops hair loss and a fall in neutrophil and platelet counts. Which statement best explains these side effects?
Answer: (B)
Principle: cytotoxic chemotherapy is non-selective for cancer cells in the sense that it targets rapid cell division. Tissues with high turnover, such as hair follicles, gut lining and bone marrow, are therefore affected. Apply this: hair loss reflects damage to dividing follicle cells, and falling neutrophil and platelet counts reflect reduced production of these cells in bone marrow. Students who think the drugs spare normal cells, or who target only resting cells, will miss this link.
Q4 · Zoology · Human Health and Disease
Which of the following is a primary lymphoid organ?
Answer: (B)
The primary lymphoid organs, bone marrow and thymus, are where immature lymphocytes differentiate into antigen-sensitive lymphocytes. After maturation, lymphocytes migrate to the secondary lymphoid organs: spleen, lymph nodes, tonsils, Peyer's patches and appendix.

In rheumatoid arthritis, the body's immune system attacks the joint tissues. Which mechanism most directly explains this self-attack?
Answer: (A)
Under normal conditions, self-reactive lymphocytes are removed in the thymus or bone marrow, or are made inactive, a process called self-tolerance. In autoimmune diseases, this control fails and lymphocytes and antibodies react against self-antigens, causing chronic inflammation and tissue damage. In rheumatoid arthritis, this leads to attack on the synovial joints. Immunodeficiency, allergy and infection each involve different mechanisms. The correct answer is therefore the failure of self-tolerance.
Q6 · Zoology · Human Health and Disease
A virus has evolved to reduce surface expression of MHC class I molecules on the cells it infects, which allows it to escape recognition by CD8+ cytotoxic T lymphocytes. Which cell type is best placed to destroy such an infected cell, and why?
Answer: (A)
Principle: cell-mediated immunity against infected cells is carried out by cytotoxic T cells, which recognise viral peptides displayed on MHC class I and need prior activation. Natural killer cells, part of innate immunity, lack a specific receptor for the virus but detect cells that have lost or reduced MHC class I, and they kill such cells without prior sensitisation. Applying this: the virus's strategy of reducing MHC class I removes the signal that CD8+ T cells need, but it creates the signal NK cells respond to. So NK cells are the best placed. Helper T cells and class-II-based recognition are the classic confusions, and the macrophage option ignores the lack of antibody in the scenario.

The Widal test is a common laboratory test for typhoid fever. What does it detect?
Answer: (B)
Typhoid fever is caused by the bacterium Salmonella typhi, spread through contaminated food and water. The Widal test is a serological (antibody-based) test: the patient's serum is mixed with Salmonella antigens, and agglutination (clumping) indicates antibodies produced against the pathogen. It therefore detects the host's immune response, not the bacterium directly.
Q8 · Zoology · Human Health and Disease
A mutation that inactivates both copies of a tumour suppressor gene such as Rb or p53 contributes to cancer most directly because:
Answer: (A)
Principle: cancer arises when growth-promoting genes (proto-oncogenes) become overactive, or when growth-restraining genes (tumour suppressor genes such as Rb and p53) are inactivated. Applying it: both alleles of a suppressor gene are inactivated, so the cell loses its normal restraint on the cell cycle and proliferates abnormally. This is a loss-of-function mechanism, the opposite of the gain-of-function change that converts proto-oncogenes into oncogenes.
Q9 · Zoology · Human Health and Disease
Papain digestion of an IgG molecule yields two identical antigen-binding fragments and one crystallisable fragment. Which statement correctly describes where antigen specificity and effector function reside?
Answer: (A)
Principle: an antibody monomer has two identical heavy chains and two identical light chains (H2L2). Each chain has a variable (V) region at the N-terminal end and a constant (C) region. Papain cleaves the heavy chains in the hinge region, giving two Fab fragments (each a light chain plus the V and first constant domain of one heavy chain, forming the antigen-binding site) and one Fc fragment (the remaining constant domains, which determine class and effector function). Applying this: the specificity lies in the Fab variable regions, and the Fc fragment carries the effector functions, so the first option is correct. The distractors correspond to reversing the variable/constant roles, misplacing the hinge's function, and misremembering the chain composition of Fab.
Q10 · Zoology · Human Health and Disease
Which of the following drugs is classed as a hallucinogen?
Answer: (A)
Hallucinogens such as LSD and mescaline distort sensory perception and produce hallucinations. Amphetamine and nicotine are stimulants, and morphine is an opioid depressant, so LSD is the only hallucinogen in the list.
